At Connections Home Care and Nursing, we tailor a support structure that meets our participants’ needs.

We work with participants, their family and Support Coordinator to ensure that participants receive supports that they are happy with and aligns with their goals and NDIS plan.

Supports can include all aspects of daily living tasks including personal care, household tasks and social participation.

We work closely with participants and their support networks to create personalised support plans to enhance independent living. Our team of passionate and highly trained disability support workers are here to provide participants with as much or as little support as participants need.

We work with the participants to create an environment where participants can lead rich and fulfilling lives. Our team of Coordinators will work closely with participants to ensure that we match our participants with the right people who share similar interest and personalities.
