Connections Home Care and Nursing is an NDIS registered disability service provider.
We provide a range of disability services that are flexible and responsive to the needs of our clients, their families and their community.
At Connections Home Care and Nursing we respect and value our clients’ choices. We work with our clients and their families to understand the support they require and how our services can assist them to meet their requirements and goals.
Through our team of highly skilled and trained disability support workers, we are able to provide all levels of care needs including complex care.
We have a dedicated team of Registered Nurses (RN) as part of our Management Team who provides ongoing training to all of our support team and oversees the provision of care to all of our clients.
Whether the support is required in home or within the community, we will work collaboratively with our clients, their family and team of supports to ensure that they are respected, valued and empowered.
At Connections Home Care and Nursing, we provide disability services tailored to our clients’ needs. We work with our clients to understand their care needs and carefully and thoughtfully match our team of skilled, professional and highly experienced disability support workers and nurses to ensure they receive high quality care that is tailored to them.
Our clients are always able to contact us 24 hours, 7 days a week, 365 days a year and will always be able to speak with their Service Manager.
At Connections Home Care and Nursing, our clients can expect support that is:

- Provided by highly skilled and trained disability service professionals. We provide ongoing training and mentoring for all of our care team by our in-house Registered Nurse.
Our team of disability support workers are able to transport clients to/from appointments or the community for various social activities (mileage charges apply).
Connections Home Care and Nursing will work collaboratively with clients and their network of supports, including allied health professionals, Support Coordinators, Case Managers or Local Area Coordinators (LACs), to ensure clients receive high quality supportive care.
